You can complete the Claimant Questionnaire through the online claims submission portal. You’ll want to be sure to include any relevant documents, including medical records, itemized bills, and any receipts from your visit.

Through your UHC Member Portal, you can track the status of your claims and access your Explanation of Benefits (EOBs) once they’ve been processed. We strongly recommend you register for your UHC Member Portal as soon as your coverage starts.

When a claim is processed, you will receive an Explanation of Benefits (also called an EOB), which will also be accessible through your UHC Member Portal. This will explain how your claim has been processed or if any additional information has been requested.
