Claim Submission

After visiting the doctor or hospital, you will need to file a claim for each new injury or illness with Point Comfort directly. Please complete the Claim Form and Accident Questionnaire (if your visit was due to an accident) and submit this, along with any invoices, receipts, and medical records you’ve received from your visit, to Point Comfort for review.

Please Note — you will need to submit your claim documents to Point Comfort within the timely filing window outlined in your policy wording. Additional claim forms for Emergency Transportation Services can be found in the FAQ section below.

UHC Member Portal — Claims Tracking

Through your UHC Member Portal, you can track the status of your claims and access your Explanation of Benefits (EOBs) once they’ve been processed. We strongly recommend you register for your UHC Member Portal as soon as your coverage starts.

  • Do I need to file a claim after every visit? You are required to file a claim by completing a claim form (and the accident questionnaire if your visit was due to an accident) once per injury or illness.
  • How do I file a claim?

    You can submit your claim to Point Comfort via email at travelclaims@pointcomfort.com. You can access the different claim forms below.

  • How can I check on the status of my claim?

    You can check on the status of your claims through your online UHC Member Portal, or by contacting Point Comfort directly with any questions you may have.

  • What is an Explanation of Benefits (EOB)?

    When a claim is processed, you will receive an Explanation of Benefits (also called an EOB), which will also be accessible through your UHC Member Portal. This will explain how your claim has been processed or if any additional information has been requested.   • I received a bill, what should I do next?

    If you receive a medical bill, you should log in to your UHC Member Portal to make sure the claim associated with the bill is on file.

    • If you do not see this claim through your UHC Member Portal, we would recommend contacting the phone number on the bill you received to make sure the doctor/hospital has your insurance information on file. You can provide them with the USA claims address listed on your ID card and ask that they submit the claim to the insurance company for processing.
    • If the claim is on file, you can download the Explanation of Benefits through your UHC Member Portal to show how it was processed. If any additional information is needed to get the claim finalized, you can submit this to Point Comfort through the online claims submission portal or via email.
    • If you aren’t sure if the claim is on file or how it’s been processed, you can contact Point Comfort directly for further assistance.
